Position Summary

The Speech and Language Pathologist is responsible for providing clinical Speech-Language Pathology services including assessment, diagnosis, program planning, therapy, counselling, consultation, and education to the referred patient, family, and mediators for the Couchiching Health at Home program.

PRIMARY RESPONSIBILITIES:

• Assessing and diagnosing communication deficits.
• Interpreting and reporting test results.
• Planning appropriate intervention programs to improve clients existing communication skills.
• Providing direct therapy on a limited basis
• Providing services based on a mediator model approach, if applicable
• Establishing programs to be carried out by a primary mediator.
• Evaluating and revising treatment programs based on reports from primary mediator and reassessment information.
• Counseling mediators, agency staff, and family members/guardians regarding their role in the management of the client.
• Consulting with other professionals regarding management of clients and acting as a team member for interdisciplinary involvement.
• Training mediators through demonstration and supervised practice.
• Providing education through in-service programs to the staff of outside agencies.
• Maintaining adequate assessment, treatment, progress, and discharge notes.
• Liaising with referring professionals, team members, and other related disciplines to ensure the highest quality of service.
• Performing administrative duties associated with patient care.
• Providing ongoing instruction and educational support for mediators.
• Providing services to the patients that meet the criteria of the Couchiching Health at Home program.
• Meeting the chart audit requirements of the service.
• Completing client/mediator contact and appropriate documentation on a bi-monthly basis.
• Making appropriate referrals to other professionals.
• Complying with his/her professional Code of Ethics, and Hospital and Departmental policies and procedures.
• Supervising Interns/Students and providing them with a high quality of clinical experience by demonstrating procedures, assigning and supervising responsibility, and providing ongoing feedback with respect to their performance.
• Participating in related continuing education programs, workshops, seminars, conferences, rounds, and clinics.
• Practicing in accordance with the guidelines established for the Speech-Language Pathology Service, as specified in the Audiology and Speech Pathology Reference Manual.
• Assisting in care and maintenance of department equipment and supplies. Assists in selection of diagnostic and treatment supplies, materials, and equipment.
• Participating in hospital and departmental quality assurance activities.

Performs other duties as may be assigned by Program Manager for Homecare Modernization

Qualifications

QUALIFICATIONS:

Education:
• Master’s Degree in the area of Speech-Language Pathology, or academic equivalent required.
• Eligibility for registration with the Ontario Association of Speech Language Pathologists and Audiologists (OSLA) required.
• Valid driver’s license required.

Experience:
• Minimum of one year of experience preferred.
• Ability to transport materials (up to 20 lbs.), or push a patient in a wheelchair.
• Ability to sustain intense listening/mental concentration.
• Ability to make frequent changes in body position.
• Good visual skills.

Competencies:
• Demonstrated strong communication skills in English, both written and verbal.
• Demonstrated ability to organize, prioritize and multi task efficiently.
• Demonstrated computer literacy.
• Demonstrated ability to transport materials (up to 20 lbs.), or push a patient in a wheelchair.
• Demonstrated ability to sustain intense listening/mental concentration.
• Demonstrated ability to make frequent changes in body position.
• Demonstrated good visual skills with attention to detail.
• Demonstrated ability to work both independently or in a team environment.
• Demonstrated ability to focus on quality outcomes.
• Demonstrated knowledge of equipment used in assessing language skills.
